WHAT ARE TONSILS?
Tonsils are small, fleshy masses of lymphoid tissue located at the back of the throat. They are part of the body’s lymphatic system and function as a first line of defence against bacteria and viruses that enter the body through the mouth or nose.
ANATOMY OF TONSILS
There are four types of tonsils:

PALATINE TONSILS
These are the most well-known and are what people usually refer to when talking about tonsils. They are situated on either side of the throat and are visible when one opens their mouth wide.

PHARYNGEAL TONSILS (ADENOIDS)

These are located higher up in the throat, behind the nose, and are not usually visible without special instruments.
LINGUAL TONSILS
Located at the base of the tongue, these are also not typically visible without special medical equipment.
TUBAL TONSILS
The tubal tonsil is a small mass of lymphatic tissue located near the opening of the Eustachian tube in the nasopharynx, which is the upper part of the throat behind the nose.
FUNCTION
Tonsils act as immune system sentinels. They contain lymphocytes, which are white blood cells that help to fight off infections. When functioning properly, tonsils ‘sample’ bacteria and viruses that enter through the mouth or nose and trigger an immune response if needed.
TONSIL INFECTIONS
Tonsils are prone to becoming infected or inflamed, conditions commonly referred to as tonsillitis.
ACUTE TONSILLITIS
Acute tonsillitis refers to a sudden inflammation or infection of the tonsils. This condition typically manifests as a painful, sore throat accompanied by other symptoms like fever.
Clinical Presentation (Symptoms)
- Severe Sore Throat: Often the primary complaint, making swallowing difficult.
- Fever and Chills: A sudden rise in body temperature is common.
- Swollen Tonsils: Visible swelling, often with a white or yellow coating.
- Headache: General discomfort and headache may accompany.
- Swollen Lymph Nodes: Particularly in the neck area.
- Malaise and Fatigue: A general feeling of being unwell.
CHRONIC TONSILLITIS
Chronic tonsillitis is a persistent or recurrent inflammation of the tonsils, lasting for extended periods, often for more than three months. Unlike acute tonsillitis, which is a short-term condition, chronic tonsillitis can significantly affect one’s quality of life over time.
Clinical Presentation (Symptoms)
- Persistent Sore Throat: Mild but nagging pain, often worse in the mornings.
- Halitosis (Bad Breath): Due to bacterial activity.
- Low-grade Fever: May be intermittent.
- Difficulty Swallowing: Especially problematic with solid foods.
- Swollen Lymph Nodes: Often tender to touch, located in the neck.
- Tonsil Stones (Tonsilloliths): Frequent formation may occur.
